You have a limited amount of time to file Form 941-X. Generally, this form must be filed by the later of: 3 years from the date you filed your original return, or 2 years from the date you paid the tax.

Form 941-X, Adjusted Employer’s Quarterly Federal Tax Return or Claim for Refund is used to correct errors on Form 941. It is a standalone form. Each line on Form 941-X relates to a line on Form 941. Form 941 is due on a quarterly basis at the end of the month following the end of the quarter.
What to do if you make a correction on Form 941?
Any corrections to Form 941, line 11a (line 11 for quarters beginning before April 1, 2020), are reported on Form 941-X, line 16. Can you file Form 941 if you made payroll error?
For example, a quarterly 941 filer would file four 941-X’s if they made a payroll error throughout the entire calendar year. Do not mail a Form 941-X for a previous quarter along with a Form 941. You have a limited amount of time to file Form 941-X.
